If your company is interested in becoming a stocking dealer or
installing dealer for P.M. Lighting, LLC products,
the first step in the process is to complete the Dealer Application
Packet. This packet includes:
* Company Information / Credit Application Form
* Credit Release Form
* Credit Card Authorization Form
Download the forms by following the link below. Complete, sign (must
be signed by a corporate officer or other responsible party),
date and return all forms by mail, email or fax to:

P. M. Lighting, LLC will process all requests within 3 business days,
allowing time to complete the necessary credit reference evaluation
and send written notification to each applicant by U. S. mail to
confirm approval or denial of the application. Upon approval, all
terms and conditions will be outlined in an acceptance letter. The
